Family Financial Information (Household Income)

We determine financial circumstances based on USDA’s definition of household income. A household is a group of related or unrelated individuals who share housing, income, and expenses. You do not have to list those who are economically independent from you or other dependents, even if you share housing.

If the student is a foster child, skip this section and complete the Foster Child information below.

Include ALL household members below, including children, students away at college, and individuals without income.

Processing delays will occur for applications containing incomplete Financial Info Forms.

EARNINGS FROM WORK

List gross income from work (wages, salaries, tips, comissions). This is not the same as take-home pay.

It is the amount earned before taxes and deductions. It should be listed on your pay stub.

If self-employed, you may report income after expenses (your own business, freelance work, farm, or rental property).

WELFARE, CHILD SUPPORT, SPOUSAL MAINTENANCE

Include TANF, General Assistance, General Relief, etc. NOTE: Food stamps and FDPIR benefits are not included as income.

GROSS INCOME

Following each person’s name, for each type of income, list the annual amount received. If no income, check NO.

ALL OTHER INCOME

Include workers’ compensation, unemployment, strike benefits, net rental income, annuities, net royalties, interest, dividend income, cash withdrawn from savings, income from estates, trust and/or investments, regular contributions from people who do not live in your household, and any other income.

If you are in the military, you do not have to include combat pay as income.

PENSIONS, RETIREMENT,SOCIAL SECURITY

Include Supplemental Security Income (SSI), Veteran’s (VA) benefits, and disability benefits.

FAQs

Can a student receive a scholarship from the original individual income credit program and from the other scholarship programs (Switcher and Low Income)?

Yes. As long as the student meets each of the specific scholarship program requirements, that student could receive a scholarship from the original individual credit program, the low-income corporate credit program and from the switcher individual credit program.

Can a student receive ESA and STO money in the same year?

Yes, they can receive it in the same year, but not the same semester. Students who received an ESA in the prior academic semester and chose not to renew their ESA account for the following semester, or students who did not accept their ESA scholarship in the current semester are eligible to receive STO funds. For verification, STOs may request that parents provide a copy of a letter from ESA stating their ESA account is closed.

Is there a limit to the scholarship amounts funded through low-income corporate donations?

Yes.

Academic year 2024/2025: $6,300 for grades K-8 and $8,100 for grades 9-12.

Academic Year 2023/2024: $6,100 for grades K-8 and $7,900 for grades 9-12.

2023 School Year Scholarship Award Report:

“Corporate Low- Income” Tax Credit Scholarship: 
80.49% ($591,772.00) scholarships were awarded to students with family incomes up to 185% of the poverty level.
19.51% ($143,413.00) scholarships were awarded to students with family incomes of 185% to 342.25% of the poverty level.  


“Original” Tax Credit Scholarship: 
0% scholarships were awarded to students with family incomes up to 185% of the poverty level.
100% ($6,808.75) scholarships were awarded to students with family incomes of 185% to 342.25% of the poverty level. 

“Switcher” Tax Credit Scholarship:
0% scholarships were awarded to students with family incomes up to 185% of the poverty level.
100% ($3,573.00) scholarships were awarded to students with family incomes of 185% to 342.25% of the poverty level.
